Appointment Fees
Payment Policy
Hariz Psychiatry PLLC is a private-pay practice and does not participate in insurance networks. A valid credit card must be maintained on file and will generally be charged approximately 24 hours before each appointment.
Upon request, a superbill may be provided for patients who wish to seek reimbursement from their insurance company. Reimbursement is determined solely by the patient’s insurance plan and is not guaranteed. Patients remain responsible for all charges regardless of insurance reimbursement.
Cancellation and No-Show Policy
Appointments cancelled or rescheduled with at least 48 hours' notice will not incur a fee.
Appointments cancelled or rescheduled with less than 48 hours' notice will be charged the full appointment fee.
Failure to attend a scheduled appointment without notice ("no-show") will also result in the full appointment fee.
Insurance companies do not reimburse missed appointment or late cancellation fees.
Exceptions may be considered in the event of serious illness, family emergency, or other unforeseen circumstances at the discretion of the practice.
Late Arrivals
Patients are expected to join appointments on time.
If a patient arrives late, the appointment may be shortened in order to maintain the schedule for other patients. The full appointment fee will still apply.
Patients who are significantly late may be asked to reschedule and may be subject to the cancellation policy.
Communication Between Appointments
Routine administrative matters, including scheduling and billing questions, may be addressed through the patient portal.
Brief clinical questions may occasionally be addressed between appointments when appropriate.
Messages requiring substantial clinical review, treatment recommendations, extensive correspondence, record review, or coordination of care may require a scheduled appointment or may be billed separately based on time spent.
Electronic communications should not be used for urgent or emergency concerns.
Response times vary and are generally not immediate.
Prescription Refills
Medication refills are typically addressed during scheduled appointments.
Refills requested between appointments generally require an appropriate follow-up appointment to be scheduled. Patients are encouraged to request refills and schedule follow-up appointments in advance to avoid interruptions in treatment.
Refill requests requiring significant review or submitted after a prolonged lapse in care may require an appointment before medication can be prescribed.
Controlled substance prescriptions are subject to additional clinical and legal requirements.
Forms, Letters, and Administrative Services
Completion of forms, letters, reports, record reviews, prior authorizations, school documentation, disability paperwork, accommodation requests, and other administrative services may require additional professional time and may be billed separately.
Patients will be informed of any applicable fees before work is completed whenever possible.
Completion of requested documentation is based on clinical judgment and cannot be guaranteed.
Coordination of Care
With appropriate patient authorization, communication with therapists, primary care physicians, schools, and other treatment providers may occur when clinically appropriate.
Time spent on extensive care coordination outside of appointments may be billed separately.
Telehealth Services
All services are provided through a secure HIPAA-compliant telehealth platform.
Patients must be physically located in New York State at the time of each appointment.
Patients are responsible for ensuring access to a private location, reliable internet connection, and a device capable of supporting video visits.
Technical difficulties may occasionally require rescheduling or conversion to another clinically appropriate format when permitted by law.
Emergencies and Crisis Services
This is an outpatient practice and is not equipped to provide emergency or crisis services.
Patients experiencing a psychiatric emergency, thoughts of self-harm, thoughts of harming others, or any situation requiring immediate attention should:
Call 911
Call or text 988
Go to the nearest emergency room
Electronic messages and voicemail should not be relied upon for urgent or emergency situations.
Treatment Termination
Either the patient or the practice may choose to discontinue treatment.
When clinically appropriate, reasonable notice and referral resources will be provided to support continuity of care.
The practice reserves the right to terminate treatment for reasons including, but not limited to:
Repeated missed appointments
Repeated late cancellations
Nonpayment of fees
Inappropriate, threatening, or abusive behavior
Clinical needs that exceed the scope of the practice
